Vietnam Entry and Exit Form (M3)

The M3 form is required for all foreign passengers arriving in Vietnam by air. This form will be distributed to you either on the airplane or upon your arrival at a Vietnamese airport. Once you have filled in the form, please hand it to the immigration officer, who will stamp your passport with a visa-on-arrival or grant you an entry stamp if you already have a pre-arranged visa. To save time, you can also download the form and complete it beforehand.

How many forms do you need

Although the form indicates that you need two copies, you only need to submit one completed M3 form upon arrival at a Vietnamese airport.

Instructions to fill M3 form

1 - Name and surname: Fill in capital letters, matching your passport details. Example: SURRESH MALIK

2 - Date of birth: Enter in dd/mm/yyyy format.

3 - Sex: Indicate male or female.

4 - Place of birth: Write both the city and country name.

5 - Nationality: Indicate both nationalities if applicable. Example: United States / United Kingdom (not American or British).

6 - Passport details: Provide your passport number, type of passport (standard/diplomatic/official), date of issue, and expiry date. For the "Issued by" section, check your passport for the details.

7 - Profession and place of employment: This should match the information provided to the Vietnamese embassy when applying for a visa. For visa-on-arrival, simply write your current occupation.

8 - Present address: This is your home address in the country from which you are traveling.

9 - Children accompanying: List all children accompanying you.

10 - Purpose of entry and exit: Indicate the purpose that applies (tourism/business/study/work/other).

11 - Name and address of contact: This might be the name of your hotel if you're a tourist, or a company name if traveling for business.

12 - Proposed duration of stay in Vietnam: Provide the most accurate estimation possible, and ensure this information matches the visa you applied for.

13 - Port of arrival and departure: Write down Arrival: (Tan Son Nhat Airport, Noi Bai Airport, etc.) Departure: (Tan Son Nhat Airport, Moc Bai border gate, etc.). You may be required to present a return flight ticket in certain situations.

14 - Done at... on...: Write down the name of the airport or border crossing through which you are entering Vietnam and sign the form.
